FIXED PRICE SURGERY TERMS AND CONDITIONS

1. The Fixed Price includes:
· Surgeon’s Fees and Anaesthetist’s fees.
· Hospital accommodation charges, nursing care and dressings.
· Hospital theatre fees.
· Diagnostic and therapeutic services (eg. X-ray and Physiotherapy).
· Drugs and Pathology.

2. The price does not include:
· MRI or CT scans.
· Personal expenditure e.g. ambulances, telephone calls or visitors’meals.

3. The Fixed Price applies only if the patient has obtained written Fixed Price confirmation from the Hospital prior to admission.

4. The Hospital reserves the right to refuse Fixed Price treatment.

5. The patient, referred by the General Practitioner, will be admitted to the Hospital under the care of a Consulant. The Consultant has the responsibility of advising that the patient is eligible for the Scheme. Although the Hospital will be collecting Specialist fees, the Consultant Specialists remain independent contractors and provide their services direct to the patient and not on behalf of the Hospital.

6. Your local Hospital will advise you, upon request, of the conditions which apply, should re-admission be required.

7. If a patient wishes, for their own personal convenience, to stay longer in the Hospital than the length of stay agreed for the procedure and is well enough to be discharged, then the Hospital rates will be incurred for the additional period.

8. The guarantees will cease to apply once the patient has been discharged.

9. Payment will be made in full by the patient by cash, cheque, banker’s draft or credit card before discharge from the Hospital.>
